Why I think that Itachi would know that Tobi wasn't the real Madara was because the amount of chakra emanating from Tobi, when the real Madara was present on the battlefield everyone knew about his presence.

As for actually answering the OP: Yes. Kishi planned Tobi being Obito from the start. There's too many parallels one can draw between Obito in Gaiden and Tobi's initial design for it to be a coincidence; Kishi deliberately drew Tobi to remind us of Obito.
