The one we fight before the DLC with the horse is a mad and mindless husk whereas the consort one is the spirit/consciousness who possessed the body of Mogh and made it his own. You can even tell by the horns that grow around the body or the fact that it uses blood magics a lot.

At first I was confused about why Radahn had Omen horns on his gauntlets because he didn’t seem like an Omen hunter kind of guy. Because usually the guys who had omen horns on their armor like to kill Omens. But then I remembered that his body was remade from Mohg’s corpse.
