Pistol Duels were fought, usually with high-quality guns made specifically for Dueling. They were highly accurate for their time, and they were often rifled in some way in the 19th century. Especially in Europe. Duels typically took place at about 40 feet, and the pistols were accurate at this range. They were deadly, specially made weapons, higher in quality than military arms of the time.

I have a budding theory that the reason duelling locations were so argued and mulled over (hence duelling grounds becoming a thing) is because anyone pissed off enough to be in a duel is going to be nitpicky and argumentative about everything. Probably why duelling had so many rigid rules overall. If you wanted out, you'd want an official excuse, if you really wanted in, you'd be arguing everything and looking for any edge. Rules allowed for either outcome, just streamlined a bit.

In the opening scenes of "The Wrong Box", a duel is taking place and when the participants are about to fire, they both move thair aim to the referee and shoot him instead.
