I think the reason deathshead didn't wait that long was, that it's been a whim of his. He knew Blazko and he wanted him to feel the pain of being responsible for the death of one of his comrades and he could do nothing to prevent it.
He grew impatient since he misjudged Blazkos loyalty and ordered the kill. (He was still not yet safe from the allied attack, so maybe that played a part as well. That's why he had about 5 Super-Soldiers surrounding him, upgraded with the new portable batteries.)
He claims he has no feelings and empathy but is actually a huge sadist, doing the dissection right in front of blazko and the survivors. If Blazko chooses you can see his delight but he makes the fatal mistake of keeping his word to let them live (at least till the incinerator fires up). But since he's a competent villain he was even ready to sacrifice one of his supersoldiers to guard them and look trough the various cameras just to be sure. Well.... less competent, rather extremely paranoid and a hermit.
If you pay close attention, he let his sadism get the better of him a second time. It's indicated trough the game-over screens that he always knew blazko had survived. The asylum he was delivered to was under deathsheads control. All of the soldiers there were his personal commandos (bearing strasses snake helix symbol). So the best guess is, that he wanted Blazko to feel even more pain, taking all the people of the asylum one by one. 14 years later no one was left and Blazko still not awake. That's also the moment Strasse felt safe enough that his legacy could never be shattered and ordered to finally kill all the remaining inmates. Yürgen (The Totenkopf-Kommando that kills the inmates and doctors) was very violent and this was certainly not a coincidence.
Then troughout the games game-over screens you can see Strasse becoming increasingly more nervous in the pictures. He even makes an utterly surprised face in the one from gibraltar. Because funnily enough, Blazkowic was not above killing civillians to get to deatshead and that realisation hit him like a truck. He knew he can no longer scar this man, and puts the sadism aside for a while following a few attempts (moon base alert, london monitor) to kill blazko for good. When that fails he deduced the location of the hideout trough his camera-network and attacked the resistance. A third time he could not contain his sadism and captured the resistance members dear to blazko that the black guard found. Instead of killing them he wanted to work on them personally, expecting that blazko would try to infiltrate his new castle again and he could repeat the same thing as 14 years ago. There was surprisingly small resistance for deatsheads main base and it becomes clear this was a setup. The resistance could unexpectedly free themselves thanks to this oversight. Then Deatsheads sadism comes a fourth time just to have Blazko be killed by his now made machine friend... Yet the prototype was hastily prepared and not yet sufficiently armoured to take an emp.
Ultimately deatshead accepted his fate even before the fight. He could have walked away to a safe location with his reserve-troops while blazko was busy with the prototype but chose not to do so... In his own words: "You perplex me captain... but you do not scare me..."
He was still a Nazi, he was sure he's better than Blazko in any way. And even if he could not best him, he died happy, knowing that the Nazis won thanks to him and his legacy and the fate of the world would be written by the victors. He was never concerned about Blazkos death or his death, but rather about his legacy and boy did he do a good job... Since the empire (albeit unstable) was still conquering much of Europe (except africa) till the 80s...

DeathHead's trait is really the patience, the first I played i was 10 seconds from getting this cutscene but I got bored immediatly
