Ive seen both animes and somewhat understood their scaling. I can somewhat see Sukuna winning, but it depends on the scenario and stuff. My opinion? Sukuna wins, but it'd be like high-extreme diff.

My opinion (No hate plis):
Sukuna wins Extreme diff (probably either way).
Deku is relative to Shigaraki. Shigaraki is Large Island-country lvl in his words (In AP/Durability/DC terms) & relativistic+ to light speed (via ratio waves).
Sukuna can be scales to light speed to ftl in R/C speed & Large country-country lvl (AP-Durability).
Arguments about Sukuna's speed:
Megumi compare Toji's speed relative to Sukuna 2f's speed. Toji is faster than Hakari, Hakari was able to react and "dodge" Kashimo's lightning (MHS feat in R/C speed). In this case, Sukuna (True Form + Full Power) would be like minimum relativistic+/light speed (ftl at max).
Kenjaku (whom is lower than Sukuna in speed terms) was able to react to Yuki's black hole & uses his antigravity to save himself, that is light speed R/C.
