I don't know but am guessing that it's less about whether feminising it is an actual insult and more about blending Elon and Melania's names to imply Elon is Trump's other wife , plus that it would seem insulting to Elon given his anti-trans track record. It's misguided since Elon will never see it and it does continue the misogynistic LGBTQA+ phobic rhetoric.
Like it or not we all still have implicit biases to weed out though , so it's still good to point stuff like this out.

It is insulting, to the target, if they happen to be the macho type. It's not that you think women are the lesser, but you know the target does, so feminizing them would be seen by them as a slur. Just like how it will annoy the hell out of the "super straight" types to call them gay.
But I agree with you, that doing such even in good faith, is still a bad thing because it perpetuates the idea that these groups are somehow the lesser even if you're saying it as a joke.

In this particular case, this particular insult might be inspired by something other than simple knee-jerk misogyny... there is after all the whole story of Elon Musk's absolutely horrific relationship with his transgender daughter -- so bad that she's officially disowned him (and any financial support/inheritance she might otherwise be entitled to.

I would not do this. I avoid using pejoratives if I can. Now, if I were to change the gender of someone's name intentionally as an insult, it would be as a way that I don't respect their name and pronouns. Nothing to do with the original gender, just basic human decency, and intentionally shirking that. Keep in mind: the original commenter was probably doing something sexist. Either way, it's just flat out rude.
