That last one is something I do, except it's with Grudges. I can't recall a single time I've gone into a Grudge room and not taken at least one hit, and I'm almost at Dead God.

For me it's a combination of not paying attention when coming back to previous cleared rooms in the floor + the spikes and OH MY GOD is it annoying af

I had a Tainted Lost run where I never found another Holy Card and the only time I took damage was to two separate spiked chests.

For me it's preparing for a specific path and then entering the wrong trapdoor

I was playing coop as tainted lost and someone walked into a room with a moving spike trap at the door while I was typing
I blame them for my pathetic death

Forgetting to get The Fool card or dropping a trinket in a boss room for The Beast
