we find a cave at the start of th ellac river where there's an enormous group of bloodfiends doing rituals. The cave is filled with blood.
Every major group of man flies is found nearby the Ellac or a source of water, in Belurat the sewers. The caretakers of the manflies are all hornsent able to use formless mother incantations. Imo, the plague was caused by the bloodfiends as revenge towards the hornsent by poisoning the water source of the land of shadows: the ellac river. Why man-flies? In my opinion is because the formless mother controls and manifest throuh the varios aspects of the crucible. So she gave the hornsent that form to reduce them into beasts, removing their horns, and making them ingredients for pots (the mold they grow on their bodies can be used to fill pots) for what they did to the shamans. I like to think the man-flies, since they grow mold on their bodies, will eventually evolve into the kindred of rot or the giant ants.

Ive heard a theory that i find compelling about this. Basically the youtuber suggested that the plague could instead be crucible influence similar to the omen and misbegotten in the lands between. They did a much better job than i could at explaining everything and i wish i remembered the video for yall.

The last bit about it not being spread to those that cared for them, could be a reference to the doctors, nurses, and clergy that would care for lepers.
Leprosy, while technically contagious, is not contagious through casual contact. So those who cared for the people afflicted would not always succumb to it themselves. Often being canonized for their charity (at least if they were part of the church).
I always looked at the fly men being an allegory to the stigma and fear of leprosy, especially considering that the afflicted were usually removed from society and quarantined. Which is similar to how the fly men are treated.

That sounds alot like the dungeater to me. Like that is the ground zero to the corruption he spreads. It's just everyone died before they could turn into man-flies
