My question is that if they knew how reincarnation works and if they tested this fact either from the start as they were getting immortality or when they tested it, how did they lose the Op-Op fruit when it came down to the last person to get immortality?

I could see it as a definite possibility. Even if you take in the likely possibilities of having seven equally skilled doctors consuming the fruit afterward or waiting for another seven to eight for the whole group to gain immortality, it is still possible.

My off the wall theory is that the elders are not humans who ate devil fruits, I think they are literal demons, who in the past killed Nika but Nika returned as devil fruit.
