As a medical doctor ( specialist in tropical medicine ) about to start a PhD in a totally unrelated subject: I concur! I have laways genuinely called PhD's 'real' doctors and not pretend ones ( like me)

One of my Professors at uni said that whenever anyone said they were a doctor he would ask whether medical or PhD. If they said medical he would always say, "Oh, you're not a proper doctor then?"

In Spanish "docto" is an adjective signifying "that who knows or that is well read/instructed", so the "r" implies (given that is the same word in Spanish) that the person curates others to the highest level on a given subject or transmit that quality to others... that who "docts" or form doctors.

Doctors in my country teach too. Our doctors teach medical students, college students, and residents.
To be a doctor, you need to finish a separate Bachelor's degree before getting a doctor of medicine degree.
If you're talking about recognition of hard work, you can't actually think a PhD even comes close to the hard work doctors do to get to that point? 100 hour workweeks. 24 hour shifts. 8 hour surgeries. PhDs could never.
